Admission Requirements

Kindergarten

  • Original NSO/PSA Birth Certificate
  • Photocopy of Parent's ID
  • 2 pcs 2x2 ID Picture (white background)
  • Certificate of Good Moral Character (from previous school if transferee)

Elementary (Grades 1-6)

  • Original NSO/PSA Birth Certificate
  • Photocopy of Parent's ID
  • 2 pcs 2x2 ID Picture (white background)
  • Report Card (Form 138) - Original Copy
  • Certificate of Good Moral Character - Original Copy
  • Certificate of Transfer Credential (for transferees)

Junior High School (Grades 7-10)

  • Original NSO/PSA Birth Certificate
  • Photocopy of Student's & Parent's ID
  • 2 pcs 2x2 ID Picture (white background)
  • Report Card (Form 138) - Original Copy
  • Certificate of Good Moral Character - Original Copy
  • Certificate of Transfer Credential (for transferees)

Senior High School (Grades 11-12)

  • Original NSO/PSA Birth Certificate
  • Photocopy of Student's & Parent's ID

College

  • Photocopy of Student's & Parent's ID

  • Senior High School Report Card (Form 138) - Original Copy
  • Certificate of Good Moral Character - Original Copy
  • Honorable Dismissal / Transfer Credential (for transferees)
  • Transcript of Records (for transferees)
  • Senior High School Diploma (for freshmen)

Important Notes:

  • All documents must be submitted in a long brown envelope with student's name
  • Incomplete requirements may delay the enrollment process
  • Entrance examination may be required for certain programs
  • Slots are limited - enroll early to secure your spot

Note: For transferees, ensure that all clearances from your previous school are settled before submitting requirements. The Certificate of Transfer Credential must clearly state that you are "eligible to enroll" and have no pending obligations.
